is set to be submitted to the Attorney General's chambers.

Health and Safety officials launched a investigation into the death of Teddy Sewell last November after an horrific accident claimed his life.

Mr. Sewell was crushed between the front end of a container truck and a 20,000-ton crane arm.

The father-of-three was rushed to King Edward VII Memorial Hospital, but it is believed he was dead on arrival.

The crane was lifting a container of construction materials when building was taking place at the Bull's Head car park in Hamilton when the accident happened.

This week a spokeswoman for the Health and Safety Department said the report into the accident was virtually complete and would be sent to the Attorney General.

"We have been looking at the root cause of the accident and any factors leading to that cause,'' said the spokeswoman.

Legislation forbids the Department from releasing the report publicly and it is being submitted to the AG's Chambers as part of the overall investigation into the accident.

"The report will go to the Attorney General for him to decide whether there is any case to answer,'' said the spokeswoman.
