Employers urged to file accident report
The Occupational Safety and Health Office is advising all employers that it is now past the statutory deadline for submitting the Employer’s Annual Report of Accidents, Occupational Illnesses and Dangerous Occurrences (OSHR 30) for the 2025 reporting year. Employers who have not yet submitted their return are required to do so as soon as possible.
Under the Occupational Safety and Health Regulations 2009, every employer must report the number of workplace accidents, dangerous occurrences, and occupational illnesses that took place between January 1 and December 31, 2025. Late submissions will be accepted until April 30, 2026.
Employers are encouraged to complete the OSHR 30 form online using the Government portal, which is the preferred method of submission. Employers can fill out the form online here. If no incidents have occurred, employers are advised to enter a zero in the appropriate boxes.
Alternatively, completed forms may be emailed to the Occupational Safety and Health Office at osho@gov.bm.
Where applicable,employers should also attach any outstanding Accident Investigation Reports (OSH 1) and Dangerous Occurrence Reports (OSH 6) that accompany the OSHR 30 submission.
