The St. George's resident, whose name was not released, was being cared for in the intensive care unit at King Edward VII Memorial Hospital.
He is believed to have been a passenger on a Suzuki motor bike driven by a Pembroke man, also 23.
At around 6.30 p.m. on Wednesday, they were in a collision with a Derbi cycle on Happy Valley Road, Pembroke.
The Derbi rider, a 33-year-old Pembroke man, suffered abrasions and bruises.
Both cycles were badly damaged.
Investigating officer P.c. Jerome Laws appealed for anyone with information about the accident to call him on 295-0011.
