Ombudsman's 15 recommendations
These are the 15 recommendations Ombudsman Arlene Brock gave following her damning report into racism dividing King Edward VII Memorial Hospital. Bermuda Hospitals Board CEO David Hill, who says they are mostly already being worked on, is due to report back by June 2008.
1) Change accreditation body to the US Joint Commission for the Accreditation of Healthcare Organisations, which offers more robust methods for data collection and ongoing follow-up.
2) Review bye-laws and regulations to ensure clarity, transparency and equitable implementation.
3) Analyse legacy blocks and cancellations to ensure best practice in allocation of Operating Room time.
4) Make doctors consult information databases or other resources when arbitrating between different views on clinical care.
5) Reconsider implementing outstanding recommendations from previous reports regarding the Department of Anaesthesia and reconsider hiring its own anaesthetists — at least to cover Bermuda's emergency needs.
6) Engage in a strategic review of Bermuda's clinical manpower needs, including which entity if any should hold the work permits of specialists who practise only at KEMH.
7) Review and rationalise its own structures and operations to strengthen its independence and leadership.
8) Clarify qualification equivalencies between different jurisdictions and establish an adequate induction programme.
9) Introduce an 'apples to apples' data collection and comparison when identifying problem doctors and alerting them to recurring issues.
10) Augment the Major Clinical Incident Policy to ensure a clear, accessible and confidential procedure in a separate complaints department. Introduce policy for disclosing incidents to patients.
11) Phase in mandatory, methodical and regular reviews of adverse events.
12) Entirely revamp the hospital's disciplinary process and consider appointing lay arbitrators on the disciplinary panel.
13) Require recruitment criteria for leadership positions to include training in conflict management and diversity.
14) Designate a person or office to be trained in and conduct ongoing audits and reports on the climate with respect to race, country of origin, language, gender and other diversity areas.
15) For hospital autopsies, the pathologist should confine his written opinion to the matters in which he has appropriate expertise.
